PreOtic Tris: Antimicrobial Activity and Enhanced Antibiotic Effect

PreOtic Tris has antimicrobial activity, enhances the effect of antibiotics and does not irritate the surface of the auricle. The synergistic effect of its active ingredients helps address multidrug-resistant bacterial otitis and prevent recurrence.

As a result of the widespread use of various antibiotics, microorganisms resistant to antimicrobial agents are increasingly encountered. Methicillin resistance or multiple resistance in staphylococci and multidrug resistance in Pseudomonas aeruginosa are often observed. In combating such microbes, an important role is assigned to agents with nonspecific antimicrobial activity, such as Tris EDTA. Gram-negative bacteria are often isolated in otitis in dogs and cats.

This type of bacteria is detected in chronic and persistent otitis because, due to the characteristics of their cell wall, they are often more resistant to antibiotics, antiseptics and disinfectants. Pseudomonas aeruginosa is one of the most common Gram-negative bacteria in such conditions. The low permeability of its cell wall makes it more resistant to most antimicrobial agents, including chlorhexidine. Other bacteria, such as Proteus spp, E. coli and Klebsiella spp, are also frequently found in otitis.

Systemic antibiotics do not reach sufficiently high concentrations in the tissues of the ear canal and are not very effective, so the use of local antibacterial agents is recommended. Antibiotic selection can often be complicated by resistance shown by these microorganisms.

Squalene is an organic triterpene molecule that is a precursor of cholesterol and all steroid hormones. It is naturally found in the skin; it is likely one of the few ceruminolytic ingredients that are safe for different ear structures and can be left in the canal without the need for rinsing.

As a ceruminolytic, squalene significantly facilitates and accelerates the cleaning procedure because it turns waxes and lipids into an easily washable emulsion. Studies have shown that this ingredient does not cause proliferative changes in the middle ear and is therefore permitted in cases of tympanic membrane perforation. Squalene can be used in different concentrations, and its ceruminolytic properties increase as the concentration increases.

EDTA, or ethylenediaminetetraacetic acid, has the chemical property of binding to metal ions and forming stable complexes that precipitate as chelates. It provokes the release of lipopolysaccharides from the structure of the bacterial cell wall, making it more permeable and consequently increasing the sensitivity of these bacteria to antibiotics.

Tromethamine, or Tris, is an alkaline buffer with pH 8 that enhances the chelating action of EDTA and therefore its antibacterial effectiveness. It counteracts the acidity of exudate formed in otitis externa, which helps enhance the effectiveness of antibiotics.

Benzyl alcohol is an important active component in PreOtic Tris EDTA. It is used as a preservative due to its antiseptic activity against Gram-positive and Gram-negative bacteria and also has a moderate local anesthetic effect, which helps control itching commonly present in otitis. It significantly enhances the action of Tris EDTA against Gram-negative bacteria such as Proteus spp and expands its antibacterial spectrum to Gram-positive bacteria.
